The Hardest Part of Perineal Massage Isn’t What You Think

Okay, so you’ve decided to do perineal massage before birth. Honestly? Good for you. It’s one of the very few birth-prep things with real research behind it, so you’re already ahead of the game. But can I tell you the part nobody actually warns you about? The massage itself isn’t the hard part. Keeping it up is.

Here’s the reality nobody puts on the box: this isn’t a one-and-done. We’re talking from around 34 weeks until your baby decides to show up — three to four times a week, five to seven minutes each time. That’s a routine. And sure, doing it here and there beats never doing it. But if you want it to actually pay off, it has to become something you just… do. Like any habit, the second it gets annoying or complicated, it quietly slides off your list. Every little hurdle between you and those five minutes is one more excuse to skip. So let’s treat this like the real commitment it is and set you up to follow through. Here’s what that actually takes.

Be honest about how you’re going to do this

Two things decide whether you stick with it: being able to do it whenever you get a quiet minute, and being able to physically reach. And if we’re honest, both of those quietly take your partner off the table.

Look — even if it’s not awkward for the two of you, even if your midwife swears it’s a sweet bonding moment, be real with yourself for a second. If the plan is to rely on him to show up three times a week, every single week, at the exact moment you’re also calm and relaxed enough to do this? That’s a recipe for missed sessions. You need to be able to handle this solo.

Except doing it solo has its own catch: reaching. At 34 or 35 weeks, while your bump is still manageable, you can probably get there with your hand. A few weeks later? Reaching yourself without it turning into a whole production gets… unlikely. Another hurdle.

So here’s where we land, and there’s really nothing to debate: you need a perineal massage tool. A good one takes the reaching problem off the table and lets you do this on your own, whenever works for you. But here’s the catch that matters — not every tool clears every hurdle. The question was never “tool or no tool.” It’s which tool makes this easy enough that you’ll actually keep going.

It’s not really about the tool — it’s about the whole setup

Tiny mindset shift that changes everything: you’re not shopping for a gadget, you’re building a little system that keeps you in the routine. Here’s what that system needs.

Something that keeps you accountable. This is the big one, and yes — there’s an app for it. The best tool comes with a companion app that talks you through each session. You pop in your headphones, tap the lesson for your pregnancy week (there are three or four a week), hit play, and just follow the voice and the music. Session’s over when the music stops. No guessing, no “wait, am I even doing this right.” And it quietly keeps track of how many sessions you’ve knocked out each week, so you always know where you stand. That, more than anything, is what actually gets people to stick with it.

An app that guides each session by voice and tracks your weekly consistency — the thing that actually keeps the routine going.

A handle you can hold without wrecking your wrist. Real talk: seven minutes, three or four times a week, adds up fast. If your wrist starts aching, you’re going to bail — nobody powers through wrist pain for weeks on end. You want a wide, ergonomic handle you can really grip, one that lets you keep steady pressure going even once there’s oil in the mix. Which brings me to…

Dealing with the oil. Perineal massage needs oil, and oil gets everywhere — the sheets, your hands, and suddenly you can’t keep hold of the thing. This is exactly why an oil channel is such a lifesaver: it’s a little groove that runs the oil straight down to the vaginal opening instead of all over your fingers. You drip it into the channel, it ends up right where you need it, and the only prep is sitting on a towel to catch any drips. Clean and repeatable — which, again, is the whole game when you’re trying to build a habit.

The right tip — please pay attention to this one. Perineal massage happens at the opening of the vagina. It is not a deep-inside thing. So anything with a pointed tip? Not your tool. Those pointed ones are made for pelvic-floor work inside the vagina — you push them in, that’s literally why they’re shaped that way. And I don’t care what the listing says about it being “2-in-1,” that doesn’t magically make it the right shape for this. What you want is a thumb-shaped tip — basically the shape your own thumb would make doing this by hand. Bonus: that shape helps you feel how far to go, and it’s built for the U-shape stretch that perineal massage actually is (press down at the base, then out to each side — every technique pretty much comes back to that same U). With Perimom you can literally see how far to insert it, because it’s marked with a color that shifts from white to purple, and the tip has enough give to match your own body.

A thumb-shaped tip, and a color that goes from white to purple so you can see exactly how far to insert it.

A shape that gets around your belly. Last thing: the tool needs to work like an extension of your arm and curve around your bump. Steer clear of anything S-shaped — that curve is a pelvic-floor thing, made for pushing pressure inward. For perineal massage you’re pressing from underneath, upward. Totally different job.

So… what should you actually buy?

Short version: only two tools out there were genuinely built for perineal massage from the ground up — Perimom and the Frida Mom wand. Between the two, Perimom edges ahead, mostly thanks to that oil channel, the thumb-shaped tip, and the coaching app you won’t find anywhere else. Does it cost a little more? Yep. But for something you’re trying to stick with for six straight weeks, that’s money well spent. The rest of the “perineal” tools just weren’t designed for this — and please, skip the vibrating, heated ones. Buzzing and heat do absolutely nothing for a U-shape stretch.

perineal massage

So here’s your takeaway, mama: commit to the massage, then stack the deck in your favor. Do it on your own, with a tool that clears the hurdles — a comfy grip, no-mess oil, a thumb-shaped tip, the right curve, and an app that keeps you honest. That’s how a good intention turns into a routine that actually makes it all the way to birth.
